A 5% waste factor is typical for continuous footings where the forms are straight and the subgrade is clean. If the footing steps for grade changes, we would take each step as its own run and add the vertical faces, which usually pushes waste to 7–8%.

Formwork is measured by contact area, not by volume. The two sides of a 12 in. deep footing give 2 x 1.0 ft x 212 LF = 424 SF of form contact, plus bulkheads at construction joints. That number drives the labor line more than the concrete does, which is why we never lump formwork into the concrete price.
If the geotech report calls for a deeper footing or a mud mat, both change the excavation and backfill quantities under 31 23 16, not just Division 03. Send the geotech with the drawings so the two divisions stay consistent.
